North Star Behavioral Health welcomes referrals from therapists, pediatricians, primary care physicians, school counselors, and other clinicians. This page has everything you need to refer a patient, coordinate care for a shared patient, or get in touch about a potential collaboration.
Refer a Patient
Use this form to refer a patient of any age who has agreed to be contacted. For minors, please list a parent or guardian as the primary contact. We'll send a gentle outreach email inviting them to connect with our intake team.
Refer a PatientRelease of Information
To coordinate care with our team regarding a shared patient, the patient will need to complete a Release of Information form. You're welcome to pull this up during an appointment so the patient can complete it in real time — it only takes a few minutes and allows us to communicate directly with you.
